NorthPoint Health & Wellness Center, a state-of-the-art multicultural, community-based medical and dental clinic is seeking a Case Management Assistant to join their team. The Case Management Assistant will work with primary care providers, clinic staff and other collaborative teams to assist in the identification of basic needs, organize activities, and collaborate with other health care team members to coordinate and implement plans of care for patients. Case management activities will include services for women that are pregnant and/or have chronic conditions including their families in the effort of linking them with health and community services/partners that will promote self-management, improve health outcomes and reduce disparity.

Location and hours:
This position is located at the NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, MN 55411. Work hours for this position will be Monday through Friday, between 8 a.m. and 5 p.m. and some Saturday shifts may be required from 8 a.m. - 12 p.m. on a rotating basis. Once clinic hours are extended there will be some evening shifts through 7 p.m.
This position is subject to the federal Centers for Medicare and Medicaid Services (CMS) Interim Final Rule on COVID-19 Health Care Staff Vaccination. As such, it requires the job offer to be conditional based on the candidate being fully vaccinated for COVID-19. “Fully vaccinated” means 14 days after receiving the second dose in a two-dose vaccination series approved by the U.S. Food and Drug Administration (FDA) on a full or emergency-use basis, or 14 days after receiving a single-dose vaccine approved by the FDA on a full or emergency-use basis. After a conditional job offer is made, the candidate must show proof of full vaccination prior to their start date. A COVID-19 vaccination card from the Center for Disease Control and Prevention (CDC), a vaccination record from your doctor’s office or a personal digital vaccination record constitutes sufficient “proof.” The candidate must also attest they are vaccinated by completing a form on the first day of employment. Reasonable accommodations to this vaccination requirement will be considered upon request by a candidate for medical or religious reasons as required by applicable law.

In this position, you will:

Assess, plan, and coordinate patient care according to established clinic policies and procedures, objectives, performance improvement activities and safety, as well as environment and infection control standards.

Work with the patient and interdisciplinary team to develop and implement an individualized clinical plan of care for the patient.

Interact with providers, including clinic specialists in areas such as optometry, social work, and nutrition to ensure comprehensive care for the patient and health outcomes are met.

Facilitate collaboration between community health educational resources and the patients to meet the patients’ health care needs.
-
Identify clinic and community resources and coordinate appropriate referrals.
-
Document and monitor patient care visits in Epic, the electronic medical record system.

Maintain accurate records and ensure confidentiality of all data collected during health promotion and prevention services.

One of the following:
-
Two years or more of responsible, paid social service experience providing direct client service.

A two-year human services certificate.

Two years or more of college coursework in the behavioral sciences.

Using electronic medical record systems such as Epic and performing data entry where accuracy is essential.
