This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing.The Housing team works closely with H+H care teams to identify patients; accesses suitable opportunities through strategic partnerships with government agencies, housing developers and nonprofit organizations; and supports patients through their progression to permanent housing. The team reports to the Assistant Vice President of Housing and Real Estate and sits within the Office of Managed Care.This Senior Project Manager will support NYC Health + Hospitals flagship medical respite program. HfH’s Medical Respite program provides interim housing and services for persons experiencing homelessness who have clinical needs that do not require acute hospital level of care but cannot be adequately served in a NYC Department of Homeless Services (DHS) shelter or on the street. Our current Medical Respite Program up to 90 days of interim housing with 24/7 staffing that allows patients with clinical needs to receive services including: on-site clinical services (i.e. wound care, PT/OT, IV infusion), care coordination and medication monitoring, and intensive housing case management with the goal of permanent housing post respite stay.CoordinationGeneral tasks and responsibilities include but not limited to:• Support medical respite referral and bed management procedures, in coordination with respite operators and H+H facilities• Support care coordination efforts with H+H care teams, including acute care facilities, Primary Care Safety Net clinics, and other longitudinal care teams, including development of systems to improve coordination and connections to care• Support development of respite policies and procedures, training, and operational guides that provide comprehensive overview of expectations of respite operators. Support respite operators to adopt policies and procedures, delegating to respite PMs when appropriate• Keep detailed notes of case conference discussion, tracking progress on follow-up• Support escalations for barriers to housing and discharge planning, including by liaising with partner staff, other members of Housing for Health team, and track housing escalation cases• Support activities related to expansion of medical respite program, including procurement and contracting procedures• Other project management for other housing, medical respite, and real estate initiatives as assigned.Data Analysis• Support day to day data management, including: analysis and reporting to monitor contract performance, goals, and quality improvement efforts; matching patients to housing opportunities; identification of patient housing barriers; and data clean up.• Merge and parse data sets supplied by all Housing for Health programs efficiently and be comfortable transforming dirty big data into actionable knowledge.• Communicate analyses clearly to lay audiences and use analysis to drive progress towards H+H housing goals with facility partners, external entities, and the Housing Location and Placement Services and medical respite vendors.• Utilize data analysis to drive coordination and policy creation with internal stakeholders, contracted vendors, government agencies, and housing providers.• Assists in designing evaluative measures including data analysis and procedures by which program efficiency and costs effectiveness can be audited and assist in evaluation audits.• Assist with tracking and data analysis for ad-hoc projects as assigned.Miscellaneous• Perform research and other information gathering activities and prepare these materials for input into grant proposals, reports, and/or meetings and conferences.• Support the Housing for Health teams needs across all programs as requested.• Manage and assist with special projects as requested by Assistant Vice President and Senior Directors• This job description is not intended to be all inclusive, and employee will also perform reasonable related business duties as assigned.Minimum Qualifications• Baccalaureate degree from an accredited college or university in Business or Public Administration, Healthcare Management, Engineering, Social Sciences, Physical Sciences or related discipline; and• Six (6) years of progressively responsible experience in project and program management, execution, and implementation, three (3) years of which must have been in the administration, management and/or execution of projects and programs of various complexities, including work in health care program planning, research, design or related area; or• Certification in Project Management (e.g., Project Management Professional (PMP) certification, Program Management Professional (PgMP), Certified Associate in Project Management (CAPM)) and four (4) years of experience as described in #2 above, two (2) years of which must have been in the administration, management and/or execution of projects and programs of various complexities, including work in health care program planning, research, design or related area.Department PreferencesPreference will be given to qualified candidates with the following knowledge, abilities, education, experience and/or skills:Educational Level• Graduate degree in public administration, urban planning, public health, or similar discipline OR• Undergraduate degree with at least five years’ experience with project management and hands on data analysis OR• A satisfactory equivalent of education, training and/or experience; and• Three years of satisfactory, full-time experience related to the objectives of the project; or• A satisfactory equivalent of education, training and/or experience.Knowledge, Skills, Abilities And Other Requirements• New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.• Interest in housing and homelessness in a healthcare setting• Experience with data analysis• Excellent written and oral communication skills.How To ApplyIf you wish to apply for this position, please apply online by clicking the "Apply Now" button.If applying online, please include your cover letter in the same file attachment with your uploaded resume.NYC Health and Hospitals offers a competitive benefits package that includes:• Comprehensive Health Benefits for employees hired to work 20+ hrs. per week• Retirement Savings and Pension Plans• Loan Forgiveness Programs for eligible employees• Paid Holidays and Vacation in accordance with employees' Collectively bargained contracts• College tuition discounts and professional development opportunities• Multiple employee discounts programs
