Program Overview
Oak Street Health@HOME delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care. Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.
Role Description
The Senior House Calls Medical Assistant/Scribe (Senior CIS/MA) will be responsible for training new House Calls CIS/MAs (working with Practice Manager/leadership to develop their onboarding plans, then leading relevant CIS/MA onboarding sessions), supporting training for other members new to the House Calls team (e.g., providers), monitoring performance of both new and tenured CIS/MAs, and providing cohort updates/trainings as needed. The Senior CIS/MA will collaborate with Practice Managers and program leadership on these tasks.
The House Calls Medical Assistant/Scribe (CIS/MA) fulfills a crucial role for our in-home care teams; supporting logistics, clinical care, and documentation in our innovative home-based care program. The CIS/MA works directly with a House Calls Provider on a daily basis to help prepare patient visits, document visits in the EMR accurately, identify and bridge gaps in care, and advocate for patients and families when patients return from the hospital or experience other challenges.
Responsibilities
Train and monitor progress for new CIS/MAs
Support training for other members of the House Calls Program who interface with MA activities or documentation
Monitor performance of existing CIS/MAs in collaboration with Practice Managers and leadership
Provide updates/ongoing trainings to CIS/MA cohort (e.g., regarding new workflows or data trends)
Remain familiar with core CIS/MA responsibilities (see below)
Prepare daily visit schedules, including routing and logistics planning
Accompany House Calls Provider on home visits
In accordance with state regulations, check vital signs, administer vaccinations, perform point of care tests, blood draws for lab testing
Assist in reconciling patient medications
Support patient and family education
Document Patient Encounters in a structured note
Include the history of the present illness, assessment, plan, and physical exam
Assign appropriate CPT and ICD-10 codes
Prepare After Visit Summaries
Consult with provider to ensure accurate and specific documentation
Improve Clinical Documentation, as appropriate
Request and review medical records
Leverage Oak Streetâs population health tools to support clinical documentation improvement in a value-based care model
Prepare for and support Daily Huddles and Clinical Documentation Reviews
Consult with provider on clinical documentation opportunities
Administrative support for provider and care team
Process and manage orders and referrals, including follow-up calls
Address tasks
Maintain supplies and equipment, including routine quality control checking (i.e. infection control measures and logs, recruitment and checks for expired medication and supplies)
Participate in daily huddles and weekly interdisciplinary team meetings to discuss patient care and program operations
Other duties as assigned

Required Qualifications
Experience training/leading a cohort of MAs and/or medical scribes
State or national Medical Assistant certification (as required by state), or graduation from an accredited medical assistant course
CPR or BLS Certification
Minimum 1 year experience as a Medical Assistant
Must pass all clinical competencies before supporting patient care, phlebotomy experience a plus
Daily access to a vehicle with valid license and insurance
Flexibility to travel throughout service area
Electronic Medical Record experience
Computer skills: Ability to quickly navigate and use multiple computer programs to include, but not limited to: Gmail, MS Word or Google Docs, Excel or Google Sheets, etc.
Bilingual (English and Spanish) a plus
US work authorization
