Join us and experience these exceptional benefits firsthand!Required Qualifications:• One year of experience practicing independently as a Nurse Practitioner• Must hold an active Nurse Practitioner (NP) license or be able to obtain one by the start date. The license should be unencumbered, with no unresolved disciplinary actions, probation, or practice restrictions.• Must have active national certification from ANCC or AANP in one of the following areas: Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care. Alternatively, must be able to obtain national certification and/or NP license in the state of assignment by the start date.• Must be capable of meeting the physical demands of the job, including lifting a 30-pound bag in and out of a car, navigating stairs, and working in various dwelling conditions. Must be able to sit, stand, and kneel to perform physical assessments.• Must have reliable transportation for completing home visit assessments.Primary Responsibilities:• Conduct in-home assessments, including evaluating past medical history, reviewing systems, reconciling medications, taking vital signs, and performing comprehensive physical exams.• Perform evidence-based practice screenings, including point-of-care testing when appropriate.• Identify diagnoses for care management and active medical treatment.• Formulate a list of current and past medical conditions using clinical knowledge, judgment, and assessment findings.• Communicate assessment findings to inform the member’s PCP of potential gaps in care.• Identify and appropriately intervene in urgent and emergent situations.• Educate members on disease processes, medications, and compliance.• Travel 85% of the time, with 15% of time spent at home.Pay range and compensation package:The salary range for California residents is $85,000 to $167,300 per year.
