The Role

We're seeking a compassionate and tech-savvy Care Coordinator & Scheduler to be the bridge between our exceptional Wellness Assistants and the families we serve. You'll play a crucial role in orchestrating care that truly works for modern families, whether they live across the street or across the country from their loved ones.

Schedule & Compensation

• Full-Time Position: Monday through Friday, 40 hours per week

• Base Salary: $39,500 - $45,500 (based on experience) annually

What You'll Do

• Coordinate Personalized Care Plans: Match families with experienced CNAs who best fit their unique needs and schedules

• Manage Dynamic Scheduling: Utilize our technology platform to ensure seamless care coordination and real-time schedule management

• Lead Client Communication: Build trusted relationships with families, serving as their primary point of contact for care planning and adjustments

• Support Our Wellness Team: Work closely with our CNAs to optimize their schedules and ensure they have everything they need to provide exceptional care

• Quality Assurance: Monitor care delivery and gather feedback to maintain our high standards of service

• Virtual Care Support: Collaborate with our virtual care team to provide comprehensive support between in-person visits
  

What We're Looking For

• Valid CNA license (minimum of 1 year of experience)

• Experience in healthcare coordination, customer service, or related field

• Strong problem-solving skills and ability to think on your feet

• Excellent communication skills and emotional intelligence

• Tech-savvy with ability to use modern scheduling and communication tools

• Genuine passion for helping seniors and their families

• Ability to work independently while maintaining strong team collaboration

Requirements

• Strong organizational and time management skills

• Proficiency with digital tools and scheduling software

• Available for occasional on-call support (rotating schedule)

• Previous experience in healthcare coordination or customer service preferred

• Valid driver's license and reliable transportation
