Bilingual RN Care Manager (Remote Flexible - Spanish Speaking)
Pair Team is looking for a Bilingual RN Care Manager to support individuals with complex medical needs and drive improved outcomes for underserved communities. The ideal candidate must be empathetic, have strong technical skills, and be fluent in English and Spanish.
Skills
- Active Registered Nurse license in California
- Care coordination or case management experience
- Bilingual in English/Spanish
- Strong technical skills and familiarity with CRM databases and Google suite
- Cultural fluency and empathy towards underserved communities
Responsibilities
- Work with individuals with complex medical needs to identify health/wellness goals
- Provide health education and medication reconciliation
- Coordinate care management services and assessments
- Collaborate with multidisciplinary care team and provide clinical interventions
- Build trust with individuals experiencing homelessness or mental health conditions
