Under the direct supervision of the oncology and infusion services manager, the Cancer Care Navigator serves as a proactive patient representative providing individualized support and guidance as patients navigate through complex cancer care issues and treatment. Responsibilities include but are not limited to the following: Delivers direct and indirect patient centered care utilizing the nursing process through assessment, data collection, and prioritizing identified patient needs. Demonstrates empathy and effectively engages with the patients and their families. Serves as the subject matter expert and educator in oncology. Expedite access to care to all patients and their families requiring cancer treatment. Works with patients and their families to cope with a cancer diagnosis and treatment. Collaborates with all members of the oncology interdisciplinary team. Serves as an educational resource and promotes awareness for the facility and community. Provides patient support in a one-on-one or group setting. Participate in quality management activities.
Ability to speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, PowerPoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.
