Per Diem LCSW/LMFT Psychiatric Consultant

Description of Position: The Psychiatricconsultant shall be employed on a consulting basis; responsible for providing assessments and counseling services to the elderly and training to the staff. The psychiatricconsultant must obtain tuberculosis (TB) clearance and health examinations within the first 7 days of employment, and must also complete a first aid and cardiopulmonary resuscitation (CPR) within the first 6 months of employment.

Qualifications:

• Provider must have a current, valid license under Board of Behavioral Sciences in the State of California
• Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), and Licensed Marriage & Family Therapist (LMFT)

Duties and Responsibilities:

• Initially assess and continue to assess participants for mental health needs in the formation of an individual plan of care by the multidisciplinary team.
• Conduct bi-annual assessment to assess case situations and presents information to develop treatment plan and other chart related documents in timely manner.
• Provide individual counseling for participants who experiences crisis issues.
• Facilitate monthly educational and counseling groups to staff.
• Work cooperatively with other community resources and referral sources to coordinate services to clients.
• Document all services performed the day service is provided to the participant.
• Maintain signed quarterly progress record in the participant’s health record.
• Assist in the reevaluation and revision of plan of care.
• Uphold the purpose and philosophy which the ADHC operation is committed to achieve as reflected in the mission statement adopted by the corporation.
• Perform duties and responsibilities in accordance with company established policy, rules and regulations.

11. Maintains appropriate state license/registration/certification for scope of practice.

12. Carry out other responsibilities as delegated.

Job Types: Part-time, Contract

Pay: $60.00 - $65.00 per hour
