Job Description

Devoted Medical Group was founded on the belief that if we treat each member like we would our loved ones, we can meaningfully improve healthcare experiences and outcomes for some of America’s most vulnerable patient populations. The Intensive Home Care (IHC) Program is at the heart of this goal: we are a highly collaborative, multidisciplinary team of physicians, nurse practitioners, case managers, social workers, community health workers, and outreach specialists united by our mission to build a better care delivery system for Devoted’s most complex members.
This position represents an amazing opportunity for an ambitious clinically focused physician ready to help grow an home-based complex care program with an interdisciplinary team. On a day-to-day basis, you will be responsible for virtual clinical oversight to a patient panel with an interdisciplinary team providing in-home care.
Responsibilities will include:
  • As the physician leading this team, you are responsible for the clinical management of the patients, with a strong focus on complex conditions including diabetes, heart failure, and obstructive pulmonary disease.
  • Your management should consider non-medical factors including social determinants, polypharmacy, and fall risk, and your care plan should be contextualized within the patients’ priorities for their own life trajectory.
  • Ensure your clinical teams are culturally centered around whole-person care by consistently putting the patients’ priorities at the center of care plans.
  • Work closely with patients, caregivers, and other stakeholders to empower patients on their journey to achieve health literacy, self-efficacy, and what matters most to them
  • Serve as the clinical backstop and accountable clinician for your patient panel.
  • Work with the nurse case manager to ensure smooth transitions of care during your patients’ vulnerable periods after an acute care episode.
  • Champion an interdisciplinary care model that utilizes the skill sets of each person on the team. For example, the community health worker is the anchor of the relationship with the patient, the social worker focuses on social determinants and behavioral complications, and the nurse practitioner is appropriately diagnosing and treating medical illness.
  • Utilize our home grown electronic health information system (Orinoco) for visits while also providing feedback on how to improve the interface.
  • Steward our patient centric and open clinical culture to our patients and colleagues.

Desired skills and experience:
  • Board-certified physician in internal medicine or family medicine with at least 2 years of clinical experience
  • Board certification in geriatrics is preferred but not required
  • Telehealth experience preferred
  • Experience managing interdisciplinary teams
  • Multi-state licensure preferred or willingness to obtain additional licensure as requested.
Salary: $228 - 315K annually
