Pine Park Health is seeking a passionate Registered Nurse to join our team in providing evidence-based, person-centered care to seniors in senior living communities.

Skills

Current unrestricted RN license
2+ years of clinical experience, preferably in geriatrics
Strong technology proficiency
Exceptional communication skills
Experience in remote roles
Familiarity with chronic care management and geriatric-related diseases

Responsibilities

Collaborate with Primary Care Providers and Medical Directors
Triage inbound communications and tasks
Provide post-appointment care and education
Conduct virtual patient assessments and develop care plans
Engage in quality improvement initiatives and research efforts
