Community Health Promoter

POSITION SUMMARY


Community Health Promoters (CHP) are well-trained peer advocates and educators who bridge the gap between their respective communities and the health care system. PPGNY’s CHPs conduct outreach and assist under/uninsured communities (particularly Black) throughout the City of Newburgh, the City of Poughkeepsie and Kingston to connect with PPGNY’s health centers and other sexual and reproductive health centers in their neighborhoods. CHPs receive comprehensive and ongoing training so they can conduct outreach and health information workshops for community members in various community-based venues. CHPs also participate in additional agency activities that promote linking Black community residents and their families to sexual and reproductive health information, resources and services. CHPs work closely with PPGNY health centers to make appointments for community members and to facilitate SRH visits to PPGNY. CHPs will provide accurate FPBP information and referrals to maximize access to enrollment and healthcare. CHPs will assist with basic program administration as needed.


ESSENTIAL JOB DUTIES AND RESPONSIBILITIES


Program Support (50%)

· Successful completion of training provided by PPGNY on various sexual and reproductive health issues, outreach, and facilitation skills. Additionally, CHPs will attend professional development training sessions as requested by supervisor.

· Conduct outreach throughout the City of Newburgh, the City of Poughkeepsie and Kingston and provide workshops at various community venues, such as community-based organizations, food pantries, laundromats, salons, as required

· Deepen partnerships with local community-based organizations and community leaders. This includes attending community coalition meetings and events when required.

· Collaborate with other Community Engagement and Education staff on outreach events, community meetings, and workshops, as scheduled.

· Work as a member of the Equity and Engagement Division and maintain high standards of professionalism. CHPs may represent the Department at selected internal and external meetings, and consortiums.

· Promote FPBP through outreach and education at community events, programs, etc.

· Encourage FPBP enrollment for community access to all methods of contraception.

· Work effectively with volunteers, trainees and other agency personnel.


Customer Service Responsibilities (15%)

· Assist clients in making appointments to PPGNY centers as appropriate; make general referrals for program participants to health and social services as needed.

· Promote and provide referrals to PPGNY’s educational and advocacy programs.


Administrative Team Support (15%)

· Assist with outreach and group education planning and scheduling.

· Assist with basic administrative duties. This may include preparing materials for workshops and outreach events, filing, setting up and breaking down special events, etc.

· Compile outreach and data collection paperwork in a timely manner.

· Complete all responsibilities according to established protocols, policies and standard practices in the areas of ADA and regulatory compliance programs such as HIPAA.


Budgetary/Statistical Support (10%)

· Completes outreach and data collection paperwork in a timely manner.


Special Projects (10%)

· Perform other educational and experience related duties or projects as required.

REQUIRED SKILLS/ABILITIES:
  • Have awareness and understanding of communities of color in the City of Newburgh, the City of Poughkeepsie and Kingston
  • Must be open and non-judgmental when encountering values, and experiences that may be different from their own.
  • Have strong interpersonal and communication skills.
  • Evenings, weekends, and travel around the City of Newburgh, the City of Poughkeepsie and Kingston is required.
  • Valid driver’s license and daily availability to an automobile to meet the travel requirements of the position.


REQUIRED QUALIFICATIONS:
  • Requires an ability equal to that usually derived from satisfactory completion of courses required for graduation from high school or GED completion.


PREFERRED QUALIFICATIONS:
  • Interested in learning and serving as a leader to teach others about sexual and reproductive health.
  • Comfort with virtual tools for program delivery or community networking.
  • Microsoft Word and other computer skills preferred.
  • Other additional languages/dialects a plus.
  • Understand the barriers that people of marginalized identities including Black, Indigenous and People of Color face in accessing sexual and reproductive health care.


TYPICAL PHYSICAL DEMANDS:
  • Requires prolonged sitting and repetitive tasks including use of a computer. Periodic standing, walking, bending. Requires lifting or moving of up to 25 pounds of programming or event materials. Must be able to occasionally walk up and down stairs. Visual acuity sufficient to perform frequent work on a computer screen and review printed reports and other materials. Requires the ability to hear and to communicate orally with others. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets, and will require reaching, grasping, pushing and pulling.


TYPICAL WORKING CONDITIONS:
  • This job operates in a professional office environment. Potential exposure to communicable diseases and other conditions in a health center environment. Requires travel to various indoor and outdoor environments to conduct programming or events. Requires flexible schedule and during peak activity periods work in excess of 7.5 hours per day and/or 37.5 hours per week.


$40,000 - $42,000 a year

All positions at PPGNY require:

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season and Covid-19) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
