Responsibilities
- Provide one-on-one virtual care visits for Pomelo patients
- Conduct behavioral health screenings and interventions
- Perform ongoing psychosocial assessments
- Develop individualized care plans
- Identify and address patient barriers to care
- Document care interactions and escalate as needed
Education
- Independently licensed as LCSW, LMFT, LPC, or LMHC
- 120 hours of clinical supervision
