Population Health Coordinator

Summary

Job Summary

This professional position is assigned the responsibility and accountability for facilitating and coordinating the assessment, planning, development, implementation and analysis of Population Health initiatives and special projects. Performs Triple Aim (cost, quality, outcomes) analysis and tasks as assigned to include, but not limited to, alternative payment model programs, grant project analysis, and clinical/quality indicators. This position serves as a resource by providing consultative assistance, team facilitation, and project coordination in support of CoxHealth goals and objectives to meet organizational mission, vision, and strategies.

Job Requirements

Education

Required: Bachelor’s degree OR An Associate's Degree AND 5 years of management experience within the related field

Experience

Preferred: Previous Quality/performance improvement and/or project management experience

Skills

Strong commitment to teamwork and ability to optimize efforts through collaboration and partnerships.

Demonstrated flexibility and responsiveness to change.

Possess excellent organizational and priority setting skills.

Possess skills in establishing and maintaining effective working relationships with members, providers, co-workers and others.

Possess ability to communicate clearly – both verbally and in writing. Able to write reports, business correspondence, and procedure manuals, to correctly and effectively present information.

Ability to work independently as well as possess team building skills in order to ensure a productive work environment and achievement of goals.

Maintain working knowledge of relevant national quality standards.

Ability to monitor and compile data and disseminate results to multiple levels of leaders and employees.

Must be able to initiate and understand research related to projects.

Proficient in Microsoft office.

Management experience is helpful that includes supervisory, budget, and patient care activities in ether inpatient or ambulatory patient care settings.
