Overview:
The Promotora I is responsible for coordinating and managing outreach activities/classes relating to diabetes, cardiovascular disease and other chronic diseases. Individual will also be responsible for co-facilitating classes in relation to the disease management program offered by AltaMed and will be responsible for the coordination and set up of educational classes based upon the curriculum requested and or required by AltaMed to meet health plan or program requirements to deliver a comprehensive self management training for P/P/C’s who have a chronic condition. Series classes on adopted curriculum including but not limited to Healthy Living or Tomando Control will be conducted by the Community Health Worker aka Promotores I with the objective and goal to move participants towards change, engagement and empowerment to become better managers of their chronic condition. Individual may be requested by Provider(s) or by the Program Director(s) or case managers from time to time to follow up on patient’s progress post classes as appropriate in order to determine and track effectiveness of program.
Responsibilities:
Individual will be responsible for facilitating chronic disease management classes in relation to disease management program offered by AltaMed, including but not limited to the coordination and set up of educational classes based upon the curriculum requested, working with clinic staff to obtain medical chart information as appropriate, ensuring there is a referral for every patient who participates in those classes, and any other information required by AltaMed to meet health plan or program requirements to deliver comprehensive classes such as diabetes, or CVD, or Asthma, etc. that will move participants towards change, engagement and empowerment to become better managers of their chronic condition. Individual may be requested by Provider(s) or by the Program Director(s) to follow up on patients progress post classes as appropriate in order to determine and track effectiveness of program and will participate in all meetings as requested by the Program Director(s) and he/she will be an active participant of the team to move towards system change and improving health disparities. In addition to the above, the Promotora will be responsible for including but not limited to the following:
  • The coordination and set up of educational classes based upon the curriculum requested and or required by AltaMed to meet health plan or program requirements to deliver a comprehensive series of classes that will move participants towards change, engagement and empowerment to become better managers of their chronic condition.
  • Participate in continuing education and training on self management and evidence based information on chronic conditions.
  • Follow up on high risk patients by face to face group setting or one on one or phone or home visit
  • Coordinate outreach activities with community-based organizations and businesses
  • Distribute educational materials and administer health assessments
  • Conduct lay education sessions on the risk factors, intervention strategies and community resources
  • Assist in Diabetes Clinic Days or other types of Intervention programs offered under the Chronic Disease Management dept.
  • Assess the participants’ health through a simple risk assessment of diabetes, cardiovascular disease and other chronic diseases.
  • Link participants to ALTAMED for primary care services and or identify and communicate with care team regarding patient's identified needs
  • Develop strong working relationships w/businesses and organizations in our service area
  • Document services and interactions
  • Work with the rest of the disease management team to provide comprehensive care to patients
  • Advocate for patients and support for stronger communities
  • Make referrals to community agencies as appropriate
  • Organize and participate in health fairs/seminars
  • Responsible for performing other duties as required assisting ALTAMED in achieving its mission
  • Works effectively and professionally as a team member.
  • Maintain data base of patient encounters (phone or visit).
  • Administer and enter outreach data
  • Participate in evaluation of program
  • Attend disease management team meetings
  • Attend other staff meetings as appropriate
  • Enter data as needed into the registry
  • Schedule patients in the ALTAMED Practice Scheduling System as needed
  • Maintain educational materialsCorder more through education committee as needed
  • Prepared to work evenings & weekends as needed
Qualifications:
  • High School Diploma or equivalent
  • Minimum 6mos to 1 year experience or internship in health care field or community outreach or community setting
  • Medical Assistant background highly desirable
