Responsibilities

The role of Records Analyst within the Record Management Unit is to ensure medical records’ data integrity and quiality for the across UC Davis Health enterprise. The Records Analyst opeartes in line with UC Davis Health compliance and established Record Management’s policies, procedures and timelines. The Records Analyst is responsible for accurately processing time sensitive requests for patient documents and imaging, monitoring patient identity and processing requests for the ‘release of information’. Other responsibilities include resolving trouble tickets, completing electronic chart corrections, resolving error logs, and providing technical, project management and unit support.

Required Qualifications

  • Ability to identify all clinical documents for patient level, order level and encounter level in the electronic chart.
  • Knowledge of all approved Document Type categories and subcategories.
  • Legal Medical Records knowledge to provide timely and accurate legal record for patient care, research, education and etc.
  • Knowledge of processes and methodologies to stay within compliance at UC Davis Health.
  • Knowledge of record integrity standards, policy and procedures.
  • Ability to apply policies and procedures regarding data integrity and legal record principles.
  • Knowledge of fine indexing protocols with an accuracy level of 95% of better required.
  • Must be able to apply federal and state regulations, as well as JCAHO standards pertaining to documentation in the patient medical record.
  • Ability to independently manage workload and prioritize according to Record Management and Document Media Health System Policy and Procedures, including processing requests for patient care within a 30 min turn-around-time.
  • Ability to perform quality assurance audits and be able to provide accurate, constructive and relevant documented feedback to peers and other members of the Record Management Unit
  • Ability to validate the document imaging work in the Onbase system and EMR, and resolve any chart corrections discrepancies in the electronic systems to synchronies the content.
  • Ability to read handwritten and transcribed documents in the medical records, interpret information, and enter complete and accurate data into a computer database and/or file.
  • Ability to prepare and batch all paper material for scanning according to protocol and document types, as well as being able to provide this training to the Emergency Departments, Surgical areas, OR, PCNs, HBCs, Inpatient settings and other members of the workforce.
  • Physical ability to sit/stand for the duration of the shift.
  • Physical ability to reach overhead, bend, squat, and kneel throughout the shift.
  • Ability to see color in order to process a color-coded medical record.
  • Reading skills and eyesight sufficient to quickly and accurately read numbers, names and bar codes.
  • Ability to move and carry charts and other items around the department and the hospital, pull and re-file charts from mobile shelving in narrow aisles.

  • This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.
