Homelessness Prevention System Manager – Full-Time
JOB ANNOUNCEMENT
Sacred Heart Community Service (SHCS), located in San José, CA, is looking for a multifaceted Homelessness Prevention System (HPS) Manager to coordinate a countywide network of community partners working together to prevent homelessness for families and individuals in Santa Clara County. This individual must be highly organized, possess an ability to work flexibly and creatively, and demonstrate superior communication skills. The ideal candidate must be equally committed to our

For over 60 years, SHCS has been one of the leading institutions addressing poverty in Santa Clara County through a strategy that combines addressing the consequences of poverty through impactful programs while building power with people to push for systemic change.
POSITION SUMMARY
The HPS manager is a core member of the leadership team for the Santa Clara County Homelessness Prevention System (HPS), providing coordination and facilitating collaboration among the participating partner agencies. The HPS is a countywide network of approximately 20 agencies that provide screening/assessments, financial assistance, legal assistance, and case management to prevent highly vulnerable families and individuals from becoming homeless. The program is one of several strategic initiatives of Office of Supportive Housing (OSH) and is coordinated by SHCS. The HPS Manager provides direct supervision to network coordination staff and supports partner agencies with technical assistance and training, including supporting the development and growth of new HPS partner agencies. The HPS Manager helps lead the development, implementation, and ongoing support of a service model that integrates best practice social services with an anti-racism and anti-oppression worldview. The HPS Manager supports system development, continuous learning, and evaluation, which includes supporting leadership development strategies for program participants, such as the HPS Lived Experience Advisory Committee. In addition, the Manager supports, develops and maintains connections between the HPS and other systems and institutions in the community and administers partner subcontracts, including invoicing and reporting. The Manager is a member of the HPS Network Coordination Team and supports direct services, as needed, including conducting pre-screens, assessments, and intakes for families seeking assistance.
DESCRIPTION OF DUTIES
Network Coordination (40-50%)
●Supports SHCS’ efforts to initiate, develop, support, and direct community collaborations with public and non-profit organizations designed to address and prevent homelessness and financial insecurity.
●Develops, implements, and evaluates program design and evaluation of the HPS service model, which includes eligibility screening, temporary financial assistance, case management, and access to legal assistance, in alignment with HPS strategies and values.
●Leads the development, implementation and continuous improvement of policies and procedures, standards, and training for case management and leadership development practices for the HPS network.
●Reviews and approves HPS cases, including providing direction regarding financial assistance, case management, and other services.
●Supports efforts to recruit, engage, and develop new community partners based on HPS
strategies and values.
●Leads the coordination of regional collaborative meetings and trainings and facilitates communication with partner agencies.
●Promotes understanding of regional and local trends in the use of evidence-based practices and data driven outcome measures as they apply to homelessness prevention strategies.
●Promotes understanding and integration of anti-racist and anti-oppression worldviews into the
HPS service model.
●Works collaboratively with SHCS staff and external evaluators to design and carry out system evaluation.
●Supports efforts to engage partner agencies in leadership development work, organizing, and advocacy.
●Supports administration of HPS contracts, invoicing, and reporting, including monitoring compliance and providing technical assistance to subcontractors.
●Supports the development, reporting, and analysis of HPS goals, outcomes, and performance metrics.
●Supervises network coordination staff, including hiring and providing structure, training, evaluation, and professional development.
Community Outreach and Network Support (20-30%)
●Develops outreach and accessibility strategies and works in collaboration with partner agencies to implement them.
●Conducts community outreach to promote the HPS, including publicly representing SHCS with community partners and at community events.
●Identifies and fills gaps in accessibility to the HPS through support of a mobile assessment strategy.
●Develops and nurtures collaborative relationships with community partners, including schools and the court system.
●Supervises outreach and assessment staff, including hiring and providing structure, training, evaluation, and professional development.
●Supports implementation of HPS services at SHCS and other partner agencies, as needed.
Includes conducting assessments and intakes with households seeking housing assistance and serving as onsite Manager, as needed.
Collaborative Leadership (20-30%)
●Works collaboratively to develop and implement efforts to ensure member involvement and leadership development within the HPS.
●Supports the development of a participant-driven HPS Lived Experience Advisory Committee to facilitate regular assessment of program effectiveness and housing policy.
●Supports efforts to engage partner agencies in leadership development work, organizing, and advocacy.
●Collaborates with SHCS’ Policy, Advocacy, and Organizing team to drive policy change related to affordable housing, tenant rights, and homelessness.
●Identifies and engages participants, including connecting people to organizing committees, conducting 1:1s, and assessing and implementing community engagement opportunities with volunteers and people receiving housing assistance.
●Engages and develops program volunteers, including recruiting, training, supervising, and supporting volunteers and interns.
●Supports the planning and implementation of organization-wide programs, events, training, and outreach efforts.
●Attends and participates in SHCS’s Managers Round Table.
●Serves on Emergency Response Team and serves as on-site manager as needed.
●Serves as liaison with community allies and partners. Attends coalition meetings and regional strategic planning meetings to nurture collaborative relationships with community members, volunteers, donors, and partners.
Agency Participation and Leadership (5-10%)
●Supports the planning and implementation of SHCS programs, events, training, policy campaigns, and outreach efforts.
●Attends staff meetings, retreats, and other team and agency events.
●Performs other duties as assigned.
REQUIRED QUALIFICATIONS AND EXPERIENCE:
●Strong commitment to our vision of ensuring every child and adult is free from poverty.
●Combined four or more years of experience in the following areas:
oDirect service work related to anti-poverty efforts, affordable housing, and/or homelessness.
oProgram management in a social service, public health, or related field.
oPublic policy advocacy and/or community organizing.
●Demonstrated understanding of regional housing and homelessness issues and policies.
●Demonstrated understanding of effective, data-driven strategies and methods to address homelessness and displacement.
●Ability to work with diverse people, communities, and cultures, with understanding of culturally competent practices.
●Excellent meeting planning and facilitation skills.
●Detail oriented, self-motivated, ability to multitask, provide quick responses and highly organized.
●Ability to work collaboratively in a team environment and to work independently with limited supervision when necessary.
●Excellent written, verbal, and presentation skills.
●Bachelor’s degree in human services, social work, public health or related field or equivalent experience.
●Computer literacy with proficiency in MS Word, Excel and client database systems.
●Valid CA driver’s license, reliable personal vehicle, and vehicle insurance or access to other means of transportation.
●Must be insurable under the Agency driving insurance requirements.
●Must complete a DMV and criminal background check.
PREFERRED QUALIFICATIONS AND EXPERIENCE:
●Master’s degree in social work, human services, public health, or equivalent.
●Experience convening community partnerships involving multiple organizations.
●Experience with nonprofit grant administration, including administering government contracts.
●Experience with managing program data, including monitoring data quality, developing reports, and analyzing program data and outcomes.
●Fluent (speaking and writing) in Spanish, Vietnamese, or Chinese.
REPORTING RELATIONSHIP:
This position is exempt, full-time and reports to the Director of Homelessness Prevention Partnerships.
COMPENSATION:
This is an exempt, salaried position ranging from $72,000 - $78,000 per year commensurate with experience.
