What you will be doing:
At SCP Health the patient's needs are always the most important priority and a positive patient experience is the expectation. For over 30 years, we have provided acute care to patients in the Emergency Room or as inpatients in the hospital. Because we know patients spend the majority of their time outside the four walls of the hospital, our Patient Engagement and Care Coordination Program has created services to support them 24 hours a day, 7 days a week, 365 days a year. We are committed to caring for them where they live, where they work, and whenever they need assistance. Whether a patient has a general health question, needs assistance locating a provider, or is sick or injured and needs help deciding the most appropriate treatment site, we are only a phone call away. Our 24/7 Nurse Advice and Triage Line is one of the many services offered under our Patient Engagement and Care Coordination Program and is an integral part of connecting with patients and helping patients get connected.

Our nurses are part of a 24/7/365 team of clinical and non-clinical Care Coordinators who can work within one of our Patient Engagement and Access Centers or remotely. The perfect nurse for us is one who is confident and capable of working independently and is an excellent communicator which means you are a good listener and can communicate information clearly and effectively.
A day in the life of a nurse on SCP's Nurse Advice and Triage line is filled with responding to incoming telephone calls, voice messages and/or text messages regarding clinical issues. You will help patients make health care decisions regarding the appropriate level of care, managing their general health needs, answering questions about tests, treatments, procedures, and healthy lifestyle changes. You will be responsible for utilizing your clinical knowledge and the nursing process to quickly identify if one of our standardized clinical protocols is needed to safely resolve the needs of the patient. - Ability to use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations.
- Must be comfortable sitting at a computer and talking on the phone for the workday.
- Provide clinical assessment and nursing triage to a wide variety of patients across the health continuum and/or health education to culturally diverse populations including COVID-19.
- Assists in directing patients to the most appropriate level of care
- Provide telephonic health information and nursing education related to COVID-19
- Assists and empowers patients to make better health care decisions
- Encourages patients to participate in other external programs or resources available to them in order to improve their health
- Work in collaboration with Tele-health providers by referring appropriate patients who may benefit from a virtual care visit
- Provide outbound follow up calls as necessary
Who/What we are looking for:
- Experience in Emergency Room, Inpatient Care, Case Management, Home Health, and/or Community Care Coordination services preferred
- Exceptional interpersonal, customer service, problem-solving, verbal and written communication, and conflict resolution skills
- Decisiveness and attention to detail as well as proficiency with technology, including computers, software applications, phone systems, etc.
- Strong computer skills with proficiency in Microsoft Office software
- Attention to details and the ability to handle multiple tasks simultaneously
- Telephone triage experience preferred but not required.
- Proficient computer skills utilizing and navigating multiple applications at one time including Citrix, Outlook, MS office, VPN, and basic computer problem solving skills.
EDUCATION (Required and/or Preferred):
- Graduate or US accredited nursing program with an active unrestricted RN license in good standing in the state of current residence
FIELD OF STUDY:
- Nursing - required
- Home Health
- Care Coordination
WORK EXPERIENCE/QUALIFICATIONS:
- Five to eight years of clinical experience required
- Excellent customer service skills including professional phone voice, empathy, and active listening skills
- Excellent oral, written and interpersonal communication skills.
- Clinical experience in and/or familiarity with the following areas strongly recommended: Emergency Room, Inpatient Care, Case Management, Home Health, Managed Care, and/or Community Care, Coordination services.
- Knowledge of the triage process
- Familiarity with call center tools and/or software, systems such as InContact, Genesys, Ayaya
- Knowledge of regulatory and accrediting standards relating to medical management and managed care
- Knowledge of medical terminology, claims processing, Home Health regulations
- Knowledge of fiscal management and human resource management techniques
CERTIFICATES AND LICENSES:
- Active, unrestricted RN licensure in the state where the candidate resides with the ability to obtain multi-state licensure is required
- Licensed under eNLC preferred (Compact license)
- May be required to obtain additional nursing licenses in other states as contractually required
