You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.

About the Role:

As the Director of Social Care Strategy and Partnerships, you will advance Cityblock's social care offerings, brand, reputation and partnerships nationally and set strategies to drive our presence and value in the communities where our members live to ensure members receive timely, high quality care and are well engaged with Cityblock.

Reporting to the SVP of Engagement, Marketing and Social Care, you will lead the development and execution of a social care and community partnerships strategy to 1) complement, support and extend our social care offerings and care model designed to address specific social determinants of health needs 2) develop Cityblock's brand and reputation nationally and in our communities; and 3) build strong relationships with third party social care vendors and community organizations and agencies, coalitions, advocacy organizations, policymakers to support coordinated care and referral pathways for Cityblock's members and 4) design social care interventions and fundamentals alongside our care model and population health teams.

You will partner with Cityblock's market leadership teams, care teams, partner success and community engagement liaisons on the ground across all markets to understand the needs of the members, how strategic partnerships may address those needs, and how our care team members can collaborate to make and manage referrals to community-based organizations. You will set a strategy and framework that the markets can execute and build upon and support best practice sharing.

You will also partner closely with other leaders across Cityblock, including but not limited to Engagement, Clinical Care Services, Operations, and Product to develop and execute strategies to round out our social care offerings through partners and better engage members and their families.

Your work will be centered around three domains:


National Strategy and Relationship Management

  • Advance the Social Care Strategy, including our use of Social Drivers of Health (SDoH) data and approach to scale across our markets
  • In collaboration with population health, care model innovation, behavioral insights and design, engagement and marketing, design a framework for scalable social care offerings, inclusive of our community partnership approach
  • Understand the landscape of social needs vendors to establish innovative partnerships and programs designed to address specific SDoH needs for our member base
  • Identify member needs and foster relationships with social care organizations that provide services to meet identified needs
  • Lead end-to-end partnership development, including vetting, contracting, scope and market prioritization, referral workflows, and implementation of projects.
  • Identify, develop, and manage partner relationships; work with partners to compose data-driven strategic goals for collaborative initiatives.
  • Maintain frequent, ongoing communication with partners to ensure opportunities are realized
  • Work with Network and Market teams to implement programs and partnership strategies, with an evaluation framework, and key performance indicators to measure progress.
  • Identifies and supports operations that enable data sharing, real-time collaboration, member acquisition and other key functions across the organizations

Local Market Guidance and Support

  • In collaboration with market leadership, design a framework for partnering with Community Based organizations (e.g. shelters, food pantries, legal services, elder services) to bolster capacity to participate in meaningful partnership with Cityblock
  • Establish a community of practice with market based community engagement liaisons to support best practice sharing
  • Develop a playbook for CBO network management and relationships for new markets
  • Advocate and lead efforts to support tooling and optimal social care referral management workflows for markets

Multi-market partnership execution and measurement

  • Prioritize partnerships based on market and member needs
  • Support care team members in troubleshooting navigation of available resources
  • Along with Provider Network strategy, advocate for a streamlined CBO partner database and relationship management workflow
  • Liaise internally to ensure that member-facing teams are on-boarded and trained effectively to be able to leverage the strategic partner network and deploy community resources appropriately.
  • Inform member outreach strategies and approaches based on information gathered from formal and informal community engagement.
  • Day to day management of multi-market strategic partnerships including referral partners, criteria for expansion or exit criteria

Requirements for the Role:

  • 8-10 years of experience in strategic partnerships, business development or community engagement with 2+ years in community centered partners
  • Bachelor's Degree in social work, human services, or related field
  • Understanding of how social determinants impact health care and experience in interfacing between health care and social care providers
  • Ability to structure partnerships and negotiate contracts
  • Excel at representing Cityblock externally
  • Deep knowledge of social determinants and the social services sector
  • Effective communicator to internal teams and external partners
  • Ability to develop collaborative relationships quickly
  • Can work in a matrix environment
  • Represents and advances the Cityblock brand in the community and in public-facing events
  • Bias towards execution

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company's equity program, paid time off, including vacation and sick leave. The expected salary range for this position is $146,761 to $186,000. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.

Medical Clearance (for Member-Facing Roles):

You must complete Cityblock's medical clearance requirements, which include, but may not be limited to, evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation that precludes you from being vaccinated against these diseases.
