About the Role:
As the Director of Social Care Strategy and Partnerships, you will advance Cityblock's social care offerings, brand, reputation and partnerships nationally and set strategies to drive our presence and value in the communities where our members live to ensure members receive timely, high quality care and are well engaged with Cityblock.
Reporting to the SVP of Engagement, Marketing and Social Care, you will lead the development and execution of a social care and community partnerships strategy to 1) complement, support and extend our social care offerings and care model designed to address specific social determinants of health needs 2) develop Cityblock's brand and reputation nationally and in our communities; and 3) build strong relationships with third party social care vendors and community organizations and agencies, coalitions, advocacy organizations, policymakers to support coordinated care and referral pathways for Cityblock's members and 4) design social care interventions and fundamentals alongside our care model and population health teams.
You will partner with Cityblock's market leadership teams, care teams, partner success and community engagement liaisons on the ground across all markets to understand the needs of the members, how strategic partnerships may address those needs, and how our care team members can collaborate to make and manage referrals to community-based organizations. You will set a strategy and framework that the markets can execute and build upon and support best practice sharing.
You will also partner closely with other leaders across Cityblock, including but not limited to Engagement, Clinical Care Services, Operations, and Product to develop and execute strategies to round out our social care offerings through partners and better engage members and their families.

We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company's equity program, paid time off, including vacation and sick leave. The expected salary range for this position is $146,761 to $186,000. The actual offer will be at the company's sole discretion and determined by relevant business considerations, including the final candidate's qualifications, years of experience, skillset, and geographic location.
Medical Clearance (for Member-Facing Roles):
You must complete Cityblock's medical clearance requirements, which include, but may not be limited to, evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation that precludes you from being vaccinated against these diseases.
