Senior Patient Engagement Specialist
We are looking for a Senior Patient Engagement Specialist to join our Chronic Care Management (CCM) team within our Product Management division. In this role, the Patient Engagement Specialist will work to serve as an extension of the provider practice by raising awareness of the CCM Program as well as educating patients, gaining their consent, and preparing them for the program. Job Responsibilities
Reach out to patients via telephone, video, and email on behalf of the primary care doctor
Educate eligible patients on the value of the program, their qualification and how the program works. Address related patient questions.
Enroll patients in a new CCM program and schedule time with the patient and the Care Manager
Interact with Enrollment Team Lead and provider practice to collaborate and share updates and insights regarding patients and patient panel
Typical Qualifications
Associates degree required
3+ years of experience required in customer service or inside sales
Previous healthcare experience preferred
Personal demeanor that expresses empathy with a patient-focused mindset and engagement
Proven adaptability in a high-volume sales space, ability to meet deadlines, and metrics
Strong communication and telephonic skills
Experience with CRM and sales systems preferred
Comfortable with a wide variety of software tools
