Social Worker (FTE)

The Licensed Clinical Social Worker (LCSW) will serve an invaluable role to ensure members’ psychosocial needs are treated with industry-leading interventions.

The LCSW will play a key role serving members across the United States while working directly with our team of clinicians, pharmacists, behavioral health specialists, care coordinators and more. This role will include direct member interaction as well as interdisciplinary team consultation, using the LCSW’s expertise to serve members enrolled in Included Health’s Case and Care Management Program (the Connected Care Program), Virtual Primary Care program, and within the scope of our other product offerings, which may include employer benefits and/or local community psychosocial navigation/coordination. Interventions may be provided by Included Health or through avenues such as local care, community resources, or other available plan benefits, with guidance by the LCSW and other care team members.

As Included Health seeks to impact both quality and cost for populations under our care, the LCSW may also collaborate with clinical leadership, using their expertise to maintain cutting-edge, visionary care to improve the health of entire populations. This may include social determinants of health (SDoH) and behavioral health intervention development/improvement, assessment of internal and external SDoH and behavioral health solutions to care for members, and ROI methodology input.

Responsibilities

  • Directly serve members, addressing psychosocial needs within the scope of Included Health’s products, including both coordination and direct member care.
  • Participate on a collaborative interdisciplinary team to deliver seamless, high quality care to complex members.
  • Use validated techniques such as Motivational Interviewing to educate and support members on various mental and emotional interventions to achieve improved health.
  • Navigate members to local medical and behavioral health providers, community resources, or health care benefits to ensure high quality care and psychosocial support.
  • Support the larger clinical team by consulting on social work and behavioral-health related questions.
  • Support our training team as needed in training other care team staff as appropriate on key psychosocial issues.
  • Assist leadership in building and prioritizing SDoH and behavioral health solutions for members.

Qualifications

  • Licensed Clinical Social Worker, with full unrestricted license in good standing with the governing board of their state and willing to obtain additional state licenses. Ability to easily obtain prior supervision documentation for licensing purposes strongly preferred.
  • Minimum 3 years clinical experience.
  • Comfortable caring holistically for complex patients with varying medical and behavioral health conditions.
  • Ability to understand cultural and socioeconomic issues affecting members and think creatively about all the resources available at a local or employer benefit level to be able to ensure members can get to the appropriate level of care.
  • Highly Empathetic. You will work with patients and their families who are going through challenging times.
  • Efficient. We are a fast growing company and we are busy! You will have lots of work to do and will be expected to meet volume goals without sacrificing quality.
  • Flexible and comfortable with working in a rapidly-changing environment. Included Health is constantly evolving, and our social workers need to be able to thrive in these types of situations.
  • Able to handle a fast-paced environment with competing priorities. You need good judgment to appropriately juggle your responsibilities.
  • Strong verbal and written communication skills. You will spend a lot of time on the phone with patients and families, therefore the ability to gather a history, answer questions, and give a great “phone experience” is critical. Previous experience with remote and/or telephonic case management is preferred.
  • Collaborate well across a multidisciplinary team with clinical and non-clinical members to deliver a seamless, top-quality care experience to patients.
  • Able and willing to work until 6:00 pm local time, with occasional weekend commitments as well.
  • Strictly follow security and HIPAA regulations to protect our members’ medical information.

Schedule

  • Flexible, Monday - Friday 8am - 5pm local time
