About the Role
As a Community Health Worker for Solace, you will be a resource via phone, email, and video for Medicare patients throughout their healthcare journey.
In this role, you will guide patients and further patient care and help them achieve their health and wellness goals. You will also lead the development of a new patient care program and ensure that the company is well-equipped to help the greatest number of people.
You must be fluent in English and one additional language to be considered for this role.

What You’ll Do
Learn the Solace systems, tools, technology, partners, and expectations, while also providing your unique expertise in every interaction.
Build strong, trusting relationships with Medicare patients, where listening and empathy are the foundation for every interaction.
Be able to identify and prioritize Medicare patients’ needs and assist them to maintain a streamlined care continuum.
Develop patient care plans to holistically address social determinants of health, i.e. food resources, transportation access, and support at home.
Build the systems of the future in working with Medicare patients.
What You Bring to the Table
Strong clinical skills with strong organizational skills to match. You can balance multiple tasks and work under pressure without sacrificing organization in your communications and documentation.
Pride in your technical savvy. You have the ability to quickly and fluently learn new systems and softwares.
Extreme bias toward action and execution. You care about forging the system that empowers better patients outcomes, and you’re not shy about sharing your feedback.
Comfort with change and ambiguity. You understand that rapid changes to the business, strategy, organization, and priorities is par for the course.
You speak a second language in addition to English.
