Introduction GRADEASTD7LOCATION OF POSITIONMDH Wicomico County Health Department, Salisbury MDMain Purpose of JobThe main purpose of this position is to provide care coordination of pregnant women and families with children up to age 19 in the Administrative Care Coordination Unit. This position will also provide outreach and direct assistance to Wicomico County Medicaid and Medicaid eligible customers to ensure that they gain access to health care and other health related services appropriately. This position will provide care coordination, client education, and linkage to resources for low income and vulnerable populations. This position will also support broad based outreach for NA/MCHP children and pregnant women. This position will report to a CHN Supervisor.Minimum QualificationsEducation: Graduation from an accredited high school or possession of a high school equivalency certificate.Experience: One year of experience in providing information on health care and disease prevention within the community.Notes• Candidates may substitute experience providing assistance to individuals in a health care or social services setting on a year-for-year basis for the required education.• Candidates may substitute U.S. Armed Forces military service experience as a non-commissioned officer in Health Services classifications or Health Services and specialty codes in the health related field of work on a year-for-year basis for the required experience.Desired Or Preferred QualificationsThe desired candidate should possess experience in the following:• working with diverse cultures• providing educational information• public speaking and making presentationsLICENSES, REGISTRATIONS AND CERTIFICATIONSEmployees in this classification may be assigned duties which require the operation of a motor vehicle.
