The Aging and Long-Term Support Administration and Home & Community Services (HCS) is looking to hire a Social & Health Program Consultant 2. Home & Community Services, as a division of the Aging & Long-Term Supports Administration, promotes, plans, develops, and provides an array of long-term services and supports (LTSS) to persons aged eighteen and older with disabilities, seniors, and caregivers. LTSS are intended to maximize independence, dignity, and quality of life of these individuals.
The incumbent in this position independently provides intensive and subject matter expertise case management and eligibility services to high-risk clients in a state mental health hospital and other psychiatric facility settings, who are financially and medically dependent upon the full range of LTSS, while complying with heightened confidentiality requirements and working in conjunction with social services specialists and state hospital professional staff. This position is essential in providing input for discharging clients to ensure they are placed in a setting for which they are financially eligible to receive services.
Some of what you'll do
The position independently monitors, reviews applications, and conducts interviews to determine initial eligibility and/or re-determine financial or health services eligibility for LTSS. The incumbent reviews program policies and program process changes in client circumstances. The incumbent may review, monitor, and possibly develop analysis of Federal and State regulations and the nexus of determinability and effects on the continuing eligibility of clients pursuant to programs and policies. The position works extensively with state hospital patients, nurses, psychologists, clinicians, etc. to improve the quality of life for this specialized clientele. Incumbents have completed an extensive training program, which includes advanced level on-the-job instruction and classroom training.
Who should apply
Those with a bachelor's degree in social work, health or social science, public administration or related field AND three years of professional experience in planning, administering, developing, or delivering social, financial, health, or chemical dependency treatment service programs;
OR
A master's degree in social work, health or social science, public administration or related field AND two years of experience as stated above in long-term care systems;
OR
One year of experience as a Social and Health Program Consultant 1;
OR
Financial Service Specialist 4 may substitute year for year for the required education
Desired Knowledge and Abilities:
• A strong background working in Long-Term Care/Institutional based medical programs offered by ALTSA/Home and Community Services is desired.
• State and Federal policies and regulations related to social, financial, and health programs.
• Principles and practices of program planning and evaluation.
• Current social and economic problems related to clientele.
• Principles of community organization and public administration.
• Program consultation, development, contract development, monitoring and negotiations.
• Work effectively with program staff and local agencies.
• Analyze and resolve complex problems related to social, financial, or health programs.
• Assign and monitor workflow of financial staff.
• Review case for accuracy, detect error trends, and identify training needs.
• Communicate with staff and other professionals to obtain and evaluate information and material to provide accurate determinations.
• Work independently, research, critically analyze, understand and interpret complex resource materials, legal documents, WAC, RCS, policy and procedures, and apply complex eligibility criteria to individual cases.
