Senior Quality Improvement Facilitator - Quality Measurement Development

The Senior Quality Improvement Facilitator - Quality Measurement Development is responsible for supporting both operational and technical support for CMS post-acute quality reporting and payment programs. You will collaborate with internal and external customers and stakeholders to create guidance and educational materials and address technical questions from healthcare providers. You will also support the standardization of data elements used across assessment instruments, updating data collection instruments, instructions, and reporting manuals. You will support contributions to environmental scans, reports, and other deliverables, such as implementation roadmap, stakeholder meetings, performance feedback, and an annual report.


Essential Functions
  • Facilitates health care activities, project participation, or review and implementation with physicians, providers, and team members, ensuring plans, tasks, and timelines meet requirements.
  • Identifies, spreads, and provides education on improvement methods and processes to providers and practitioners and assists in implementation.
  • Develops and maintains activity and/or project plans.
  • Provides regular updates to Company management on activity and/or project status.
  • Researches and gathers technical and clinical data to support activities and/or project initiatives.
  • Performs analysis on the results of the data collection.
  • Based on the outcome of the analysis, determines areas for improvement.
  • Provides recommendations and feedback to project participants.
  • Assists in the development of project materials and intervention tools.


Requirements
  • Four-year degree in business, health care, IT, and/or equivalent training or experience
  • 3-5 years in a related field
  • Demonstrated experience in healthcare quality improvement, quality measurement and reporting, measure development, healthcare information management, and/or healthcare information technology (HIT)
  • Advanced knowledge and experience working with Windows, all Microsoft Office products, and other PC-based software products
  • Intermediate knowledge and experience with database applications, data trending/graphics, and reporting tools
  • 5 years of experience preferred working in direct collaboration with providers, population health or public health, and/or health care quality improvement
  • In Colorado, the estimated salary range is 84k - 99k annually.  Individual offer amounts will be determined based on a number of factors to include location, background, experience, and internal equity. 


Knowledge of the following healthcare quality principles (Preferred):
  • Prefer prior experience in a hospital or post-acute care setting (ex: inpatient rehabilitation, SNF or Nursing Homes, home health, hospice, or LTCH) and/or prior experience with CMS’ QIES/iQIES or other quality improvement programs
  • Experience with or understanding of CMS’ Quality Reporting Programs or Quality Measure Development is preferred
  • Quality Measure Development, CMS Quality Reporting Measures
  • Healthcare services, healthcare organizations, and programs
  • Information management processes, electronic health records, and healthcare information technology
  • Quality improvement concepts, principles, and process tools
  • Basic statistics, data collection techniques, data analysis, and graphic representation of data
  • Effective communication both written and verbal
  • Facilitating Change/Group process facilitation
  • Stakeholder relationships: building and maintaining skills (required)
  • Effective communication, both written and verbal
  • QI Process: Focus-Analyze-Develop-Execute/Plan-Do-Study-Act
  • Creating a graphic representation of data factors
