Telehealth Nurse Practitioner 11am-7pm EST

Twin Health is looking for a Senior Telehealth Nurse Practitioner to join our dynamic team, empower members to reverse and prevent chronic metabolic diseases, and advance personalized data-driven care.

Skills

Clinical experience in remote care management
Board-certified in family or internal medicine
Exceptional communication and problem-solving skills

Responsibilities

Engage with healthcare professionals to provide treatment and guidance to members
Assess symptoms, treat, and collaborate with members' primary care providers
Deliver care utilizing telemedicine and document in Twin's clinical platform
Manage high-risk populations and understand social determinants of health
Collaborate with medical team to provide excellent customer service and care
Provide patient education and promote healthy habits

Education

Advanced Practice Registered Nurse license in multiple states
