In this vital role, you’ll provide both group and individual Diabetes Self-Management Education and Support (DSMES) to patients and their families, play a crucial part in coordinating these services, and ensure they are effective in helping individuals navigate their diabetes journey. With a dedicated focus on improving diabetes management, you’ll oversee planning, implementation, evaluation, and the daily operations of DSMES. Your expertise will also extend to offering Medical Nutrition Therapy tailored specifically for patients with dietary needs. A minimum requirement for this role includes a Bachelor’s degree in foods and nutrition and two years of recent diabetic education experience. Additionally, the ideal candidate will hold a Registered Dietitian credential and possess or commit to obtaining the Certified Diabetes Care and Education Specialist (CDCES) certification within the first six months of joining.
