Community Health Worker - Chesterfield, Virginia

Waymark is a healthcare company focused on bringing the best healthcare to people with Medicaid benefits. They are seeking a Community Health Worker to provide social support, advocacy, and navigation for patients in low-income communities.

Responsibilities

  • Attend a 2-3-week long paid training program
  • Conduct needs assessments for patients
  • Help patients with health-related social issues
  • Create a directory of community resources
  • Manage relationships with healthcare providers
  • Expand knowledge of community resources
  • Accompany members to medical appointments
  • Document patient encounters
  • Support outreach to eligible individuals
  • Participate in care team huddles

Education

  • Community Health Worker certification (preferred)
  • Experience conducting home visits and outreach (preferred)
