Responsibilities

Attend a paid training program
Meet patients to conduct needs assessments
Help patients with health-related social issues
Manage relationships with healthcare providers
Expand knowledge of community resources
Accompany members to medical appointments
Document patient encounters
Support outreach to eligible individuals
Participate in care team huddles
