Enrollment RN
WelbeHealth PACE program seeks an Enrollment RN to assist seniors in accessing comprehensive medical care and community-based services. The role involves conducting nursing assessments, collaborating with interdisciplinary teams, and ensuring program eligibility.

Responsibilities
- Conduct initial nursing assessments in participants' homes
- Track and oversee the annual State level-of-care review process
- Collaborate with the interdisciplinary team and outreach & enrollment team
Education
- Graduate of an Accredited School of Nursing, B.S. Nursing preferred
- Unencumbered RN License in state of practice
