Skills
Strong clinical assessment and critical thinking skills

Prior experience in senior healthcare or PACE preferred, but not required
Responsibilities
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each participant's unique needs
Conduct comprehensive assessments, administer treatments, and provide health education to participants and their families
Conduct thorough assessments to assess eligibility and guide seniors and their families through the enrollment process for our PACE program
Provide skilled nursing care to seniors, perform home assessments, develop personalized care plans, and coordinate services to meet the evolving needs of our participants
Education
Graduation from an accredited nursing program
