Position Summary: Evaluates participants, administers physical therapy treatments, records progress and instructs participants and/or family in care as part of the PACE program. Supervises work activity of aides, assistants and students. Works in the PACE Center and in participants’ homes. Conducts initial assessments and periodic reassessments, develops plan of care, coordination of 24 hour care delivery, regularly informing the interdisciplinary team (IDT) of the medical, functional, and psychosocial condition of each participant, remaining alert to pertinent input from other team members, participants’ caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies.

REQUIREMENTS

  1. Bachelor’s degree in Physical Therapy if obtained prior to the year 2000; Master’s degree in Physical Therapy if obtained after the year 2000.
  2. Knowledge of clinical competency with the geriatric population.
  3. Current Licensed Physical Therapist to practice in the required state.
  4. Meet a standardized set of competencies for the specific position description established by PACE and approved by regulatory agencies before working independently.
  5. 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
  6. Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  7. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
