Hospice Patient Care Coordinator

Description

The Patient Care Coordinator is responsible for facilitating the administrative tasks of the dedicated clinical team that provides hospice care to patients in their homes and assisted living communities. The Patient Care Coordinator will work independently with assigned tasks while working in a collaborative team environment. This position will work on-site in the Capstone Hospice headquarters located in Peachtree Corners, working Monday - Friday, 8:00am-5:00pm. The position will report directly to the Patient Care Manager of the dedicated clinical team. See job description for more details on the essential functions of the role.

Candidate Application Process:

To be considered, candidates should apply online through the Capstone website or Indeed by completing an application and uploading an updated resume. Capstone Hospice will review all candidates and will respond with kindness via email. Candidates selected will go through an initial telephone interview and will be contacted via text messaging to schedule, please opt in to accept text messages. After an in-person interview, candidates for consideration will be subject to a background check, pre-employment physical and drug screening.




Requirements

Patient Care Coordinator Candidates must meet the following qualifications and experience to be considered:

  • High school graduate or equivalent (GED).
  • Prefer 1+ years' experience in Hospice administrative services. 

To be successful in this role candidates should have the following knowledge & skills:

  • Experience using Microsoft products to include Outlook, Word, Excel, Teams
  • Experience using EMR/EHR cloud-based software, Wellsky a plus. 
  • Knowledge of Medicare, Medicaid and third part reimbursement requirements. 
  • Strong organizational skills.
  • Strong attention to detail. 
  • Knowledge of Hospice policies and procedures. 
  • Ability to exercise independent judgement. 
  • Acceptance and adaptability to different social, racial, cultural and religious modes. 
  • Ability to respond with a sense of urgency. 
  • Ability to effectively manage high levels of stress. 

Essential responsibilities of the role:


DME Ordering/Pick-up

Follow up and confirm of RN requests & Orders

Management / review of DocuSign documents

Review and assign incoming faxes


PCC/PCM Meeting Once a Week to Review:

  • Admissions, Discharges, & all LOCs: 
  • DME Complaints
  • Review Payroll 
  • IDG Agenda

Admissions:

  • Complete patient admission checklist within 48hrs (AND DME, etc.)
  • Create facility binders for applicable facilities within 48hrs

Discharges:

  • Complete discharge checklists within 48hrs, all discharges must be complete for Billing
  • Full discharge with HIS to be completed within 72 hours of discharge
  • Complete Discharge Audit with 7 days of Live Discharge

Continuous Care & Other LOCs

  • Complete Level of Care Changes as soon as they occur, limit delay as much as possible
  • Coordinate care, payor checks, DME, etc. in the event of a level of care change
  • CC notes to be scanned and uploaded into console as a complete packet; RNCM should be sending copies of notes daily

IDG Responsibilities:

  • Create & Update IDG Agenda (Admissions, Discharges, Falls, etc.)
  • Run Active Patient Summary, reconcile & check for accuracy
  • IDG Agenda to be updated & brought to weekly PCC/PCM Meeting
  • Print IDG Agenda & Sign-In Sheet
  • Infection and fall report to be attached to agenda
  • Send a copy of IDG Agenda with NPs for F2F purposes
  • Run the certification report to assure that all certifications have been signed prior to the MD leaving
  • Certifications & updated plan of cares should be printed and placed in RNCM mailbox by Wed at 12 of following week (keeps facility binders updated)
  • Verify IDGs Comprehensives are signed & locked

CNA Responsibilities:

  • Maintain accurate CNA schedules and assignments in Consolo
  • Complete and send CNA schedule by end of business day Friday
  • In the event of a CNA callout, arrange for coverage as soon as possible
  • Reconcile CNA timesheets on a daily and weekly basis
  • CNA payroll must be submitted to the PCM no later than noon on payroll Monday
  • Reconcile the Visit Statistic report on a weekly basis

Medication Fills/PDC Reports:

  • Run med fill exception report and correct weekly; must be complete for end of month billing
  • Gather & send PDC reports to appropriate MD
  • Reconcile active patients against Active Patient Report

Respite/GIP/IPU: 

  • Coordinate LOC with the RNCM and MSW
  • Prepare and send records for patients going for respite or IPU stays
  • Confirm receipt of documents with receiving facility

End of month billing:

  • Reconcile billing specialist emails within 24 hours of receipt; complete by month end
  • Med Fill exception report completed by month end
  • Complete all LOCs for End of Month Billing
  • Reconcile aide visits (Visit Statistics)

DME:

  • Coordinate DME request for timely delivery
  • Enter all DME orders are in Consolo
  • Communicate delivery & ETA information to the RNCM
  • Follow up to ensure DME was delivered
  • Arrange for DME pick up within 24 hours of death/discharge (same day if possible)
  • Arrange for DME pick up when no longer in use
  • Follow up to be sure that DME has been picked up 
  • Reconcile patients in Hospice Cloud Pro monthly against Active Patient Report

Receptionist backup up and sub when receptionist is absent during normal business hours. 

Other duties as assigned. 


Position Factors:


Physical Requirements: 

  • Ability to work in an office setting, sitting at least or more than 50% of the time. 
  • Close work with finger dexterity, adequate hearing and vision (with or without correction).
  • Cubicle office setting to provide a collaborative environment with other coordinators. 
  • No more than lifting of 25lbs.

Mental Requirements: 

  • Ability to work in a collaborative environment while making independent judgments.  
  • Flexible and able to cope with the changing demands of the position. 

Risk Exposure to Bloodborne Pathogens: 

  • Low risk.
