About us:Foxdale Village, a life plan community guided by Quaker values, is a non-profit Continuing Care Retirement Community (CCRC) located in State College, PA. Foxdale was founded in 1985 by members of the State College Friends Meeting and is founded on the values of treating all with dignity and respect. Foxdale is regarded as a premier healthcare facility both locally and nationally and has been voted "Best Retirement Community in State College, PA" now for 10 years consecutively. Foxdale Village includes 57 apartments and 148 cottages and offers a range of health services with 46 beds in skilled nursing for short and long-term care, two memory care neighborhoods, and 55 beds in personal care, all with private accommodations. We currently have an opportunity for a new Healthcare Support Specialist to join our team.
We are hiring for a Full-Time position and expected work 40 hours per week. This positions typical work schedule would be Monday through Friday, 8:00am – 4:30pm.
Job Summary:The Healthcare Support Specialist works under the direction and supervision of the Medical Home Supervisor. Supports the Medical Home's day-to-day operations, including checking residents in for appointments, taking vital signs, obtaining prior authorizations, and performing other administrative tasks as needed. Performs services for the resident as necessary to maintain the resident's comfort within the Foxdale Community.

Essential Job Functions:
1. Support Resident needs on the Foxdale Campus:
a. Assists with personal care tasks, including bathing, dressing, grooming, feeding, toileting, routine catheter and colostomy care, ambulation, transfers, and positioning.
b. Performs simple dressing changes that do not require the skills of a licensed nurse.
c. Assists with physician-ordered medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse. Typically, this involves reminding the residents of the specific times to take their medication.
d. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist. (i.e.: Range of motion exercises, Home exercise program Reports changes in the resident’s condition and needs to the Registered Nurse.)
e. Performs household services essential to health care at home, including, but not limited to: Meal preparation/feeding, Laundry, Light housekeeping, Mail management, filing, etc.
f. Understands, accepts, and responds to the emotional needs of each resident.
g. Meet with prospective residents and family members to provide information on Home Care services, including intake paperwork and informational materials.
h. Process referrals to additional community health care resources for transition periods.
i. Ensure a safe home environment.
2. Supporting Medical Home needs:
a. Foster a collaborative approach in working relationships with other Health Services staff as well as residents, their families and friends, and other departments in order to ensure smooth provision of health services, health promotion, and continuity of care.
b. As one function of the Medical Home serves as a home base or gateway for medical needs, assist residents in arranging appointments with providers external to Foxdale, as well as ancillary services, including pharmacies, labs, and medical tests.
c. Participate in the assessment of residents' care needs.
d. Participate in the evaluation and prioritization of resident requests to see the physician.
e. Assist the physician in coordinating the day-to-day operations of the Medical Home.
f. Provide or identify sources of home health care for apartment residents in accordance with established policy.
3. Facilitating Resident Moves
a. Work with Foxdale interdisciplinary team to implement customized move plans and timelines for all resident projects.
b. Prepare for projects by reviewing projects, creating estimates for residents and/or residents’ families.
c. Responsible for loading/unloading supplies before and after projectsd. Provide the highest level of resident service.
e. Assist resident in determining which items to keep, donate, gift, sell, or discard and maintain detailed notes on decisions made.
f. Professionally pack all items, coordinate with Foxdale staff, and assist with unpacking and getting the resident settled in their new location.
4. Overall essential job Functions
a. Assume responsibility for completing and maintaining appropriate records and documentation in a timely and legible manner.
a. Maintain security of medical records and confidentiality. Communicate effectively with all members of the interdisciplinary team.
b. Maintains a safe resident environment and identifies and reports any suspected vulnerable resident abuse, neglect, or financial exploitation.
c. Maintains resident confidentiality; treats residents and families with respect.
d. Participate in the auditing of apartment resident charts for accuracy and completeness.
e. Uphold the rights of residents. Communicate effectively through written and non-written methods to assure continuity of care and efficiency in work responsibilities.
f. Demonstrate usage of proper body mechanics with safe lifting and transfer techniques.
g. Respect individual abilities, knowledge, and skills of residents, staff members, and families.
h. Maintain a neat and appropriate personal appearance.
i. Maintain punctuality and regular and predictable attendance to meet the daily medical needs of the residents.
Requirements
Minimum Qualifications:
Education: The Completing a formalized health-related program of study and receiving an associate’s degree or higher is preferred.
Experience: Minimum of one year of work experience in an office or healthcare setting, and currently certified as a Certified Nurse Aide. A licensed practical Nurse is preferred.
Language: Must be able to communicate effectively in English.
Math: Must be able to complete tasks using basic mathematical skills.
Computer: Must be able to use word processing and spreadsheet software and the Internet to complete tasks. Must be familiar with email. Other computer knowledge, such as use of Excel spreadsheets, is desirable.
Training: Certified Nursing Aide; Licensed Practical Nurse preferred.
