LPN (Licensed Practical Nurse) - job 1 of 2

As an LPN at Phoenix Home Care & Hospice, you will be responsible for reviewing and revising the care plan, delivering nursing care in accordance with the physician’s treatment plan, and closely observing and reporting any reactions to treatment or changes in the patient's condition. Your role will include educating clients and their families about the treatment plan and participating in quality assessment activities and team meetings.

The work schedule for an LPN at Phoenix Home Care & Hospice is typically Monday through Friday from 8 am to 5 pm, allowing for a balanced work-life experience. Additionally, there is a weekend rotation approximately every 4 to 6 weeks, ensuring you have the opportunity to enjoy time off while providing essential care to patients.
