Village Medical at Home, an extension of the physician-led, team-based platform developed by VillageMD and Village Medical, offers complete primary care services in the home. The Village Medical at Home model offers these services across a continuum of clinical needs, from wellness and quality documentation to the management of transitional care and complex medical needs. Our Village Medical at Home team is passionately committed to delivering coordinated, patient-centric care. 

The Village Medical at Home Care Coordinator is an integral part of the team which shares responsibility to improve quality of care and facilitate a positive experience for patients, families, and caregivers. This individual works specifically with high acuity patients that were recently discharged from an inpatient setting.

How you can make a difference

  • Schedules transitional care management visits and weekly follow up visits for patients discharged from the hospital.
  • Appropriately documents patient outreach & contact within CRM software.
  • Works to ensure patients have everything they need to prevent a readmission and coordinates services and resources if needed. Serves as a main point of contact for patients and ensures a positive patient experience.
  • Assists patients by providing information and services.
  • Uses the electronic medical record proficiently; transmits information electronically.
  • Works collaboratively with adjacent Village departments to coordinate patient handoffs and comprehensive care

Experience to drive change

  • Medical Assistant Certification required
  • Must have current BLS certification for healthcare providers through American Heart Association or Red Cross
  • Must have High School Diploma
  • Experience in primary care, family practice, internal medicine, urgent care or ER preferred
