Responsibilities

A Community Health Worker (CHW) serves as a critical liaison between health services and the community, working to improve public health by providing education, support, and resources. They conduct outreach, advocate for community needs, facilitate access to healthcare services, and provide culturally appropriate health education. CHWs play a vital role in helping individuals navigate the healthcare system, promote preventive health behaviors, and support disease management. Their work often includes home visits, organizing community health events, and collaborating with healthcare providers to address health disparities and improve overall community well-being.

 

  • Assist individuals and communities to adopt healthy behaviors.
  • Conduct outreach for medical personnel or health organizations to implement programs in the community that promote maintain and improve individual and community health.
  • May provide information on available resources provide social support and informal counseling advocate for individuals and community health needs and provide services such as blood pressure screening.
  • May collect data to help identify community health needs.
  • Will conduct Home Health visits

 

**This position pays $23.00**

 

Dignity Health now offers an Education Benefit program for benefit-eligible employees after 180 days. This program provides debt relief and student loan assistance to help you achieve your goals. Full-time employees can receive up to $18,000 over five years, while part-time employees can receive up to $9,000
