Acentra seeks a Service Support Facilitator to join our growing team.

Job Summary:

The CSED Waiver Service Support Facilitator is vital to the program's success by conducting initial and annual assessments which are used in determining medical eligibility and levels of Wrap Facilitation for program members. These are crucial for the member's planning team in guiding their Person Centered Plan of Care. The facilitator will assist in the process of redetermining medical eligibility through initial and annual assessments. 

*Position is remote within the State of West Virginia or bordering West Virginia.

 

Job Responsibilities:

  • Schedule, conduct and appropriately document member assessments within prescribed timelines
  • Enter assessment data into the web system
  • Participate in inter-rater activities to ensure consistency in the administration of the assessment instruments and adherence to assessment protocols
  • Participate in monthly Waiver staff meetings, quarterly all-staff meetings and other required training and meetings as necessary
  • Support in overseeing department procedures and participating in quality assurance/improvement initiatives
  • Conduct educational sessions for family members and agencies, covering policies related to assessments
  • Complete satisfaction survey with a focus on achieving positive results from respondent feedback
  • Other duties as assigned

Required Qualifications/Experience:

  • Bachelor’s Degree in Human Services (preferably social work, psychology or counseling). Masters degree and license preferred
  • Minimum of 2 years of experience with the population and possess knowledge of program policies and procedures as well as other programs/services available to program members

 

Preferred Qualifications/Experience:

  • Medicaid/Public Sector service experience is also preferred
  • Proficient understanding of person-centered planning principles
  • Familiarity with various computer applications and adept at navigating multiple screens
  • Demonstrated ability to uphold confidentiality standards and ensure HIPAA compliance when evaluating pertinent issues
  • Strong organizational skills are essential
  • Exhibits a sense of urgency to meet deadlines effectively

Compensation 

The pay range for this position is $51,480-57,364.80.

“Based on our compensation philosophy, an applicant’s placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.”
