GENERAL STATEMENT OF DUTIES:
The Client Care Coordinator will provide essential support in the day-to-day operations of Root House, ensuring clients receive the necessary services and support for their recovery journey. This role requires direct interaction with clients, facilitating both group and individual sessions, managing appointments, and coordinating resources to create a comprehensive care plan. Additionally, the Client Care Coordinator will play a key role in assisting with discharge planning to ensure clients have the tools and support they need as they transition out of the program. The ideal candidate will be compassionate, organized, and knowledgeable in substance use disorder services, with a strong ability to communicate and collaborate effectively with clients, staff, and external service providers. The Client Care Coordinator will adhere to all rules and regulations of F&CS, OHMHAS, and COA standards, including crisis intervention and regular consultation with supervisors. The position requires the ability to respond appropriately to crisis or emergency situations, and the candidate must have at least basic training in crisis intervention.
ESSENTIAL RESPONSIBILITIES:
1. Provide support and guidance to clients through group and individual sessions.
2. Assist clients with discharge planning and ensure smooth transitions to aftercare or independent living.
3. Collaborate with clients, treatment teams, and external providers to create a clear post-program care plan.
4. Monitor client progress and ensure necessary resources (housing, employment, continued counseling, etc.) are in place before discharge.
5. Schedule and coordinate appointments (medical, therapy, probation, etc.) and ensure they are approved by the Program Manager.
6. Facilitate referrals to external services (e.g., probation assessments, medical providers) and ensure follow-through.
7. Assist clients with accessing community resources (food stamps, grocery orders, housing, transportation, etc.).
8. Ensure clients have access to ongoing recovery support services (outpatient therapy, sober living, job opportunities).
9. Respond to crisis situations, provide immediate support, and follow crisis management protocols.
10. Complete Needs Assessments at admission, 45-day, and discharge to evaluate client progress and needs.
11. Document client progress and ensure compliance with confidentiality and program standards.
12. Collaborate with internal teams (counselors, social workers, probation officers) to ensure coordinated care.
13. Organize and facilitate weekend group sessions, ensuring client engagement and support.
14. Fulfilling resident monitor duties as needed.
15. Provide accurate daily shift reports and communicate any client concerns to supervisors.
16. Attend meetings and training to stay updated on policies, procedures, and best practices.
17. Maintain accurate records in compliance with agency policies and regulatory standards (F&CS, OHMHAS, COA).
18. Comply with all agency policies, state/federal requirements, and educational/certification standards.
19. Perform other duties as assigned.
Requirements
QUALIFICATIONS:
1. Chemical Dependency Counselor Assistant Licensure required.
2. The CDCA is required to have the following skills: clinical interview methods; individual, and group therapy methods.
3. Ability to develop and implement treatment plans.
4. Skills in crisis intervention and emergency services.
5. Specialized knowledge of the substance use field.
6. The ability to communicate effectively verbally and in writing.
7. De-escalation skills.
8. The ability to establish and maintain clinically effective relationships with clients.
9. Ability to meet agency requirements for criminal background check.
10. Must possess a valid Ohio driver’s license in accordance with County policy and must maintain motor vehicle liability insurance, must maintain continuing eligibility under the existing County driver eligibility standards.
11. Ability to function as part of an organization by working with others and by adhering to established policies and procedures.
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Valid Ohio Chemical Dependency Counselor Assistance Licensure.
MINIMUM EXPERIENCE REQUIREMENTS: Be knowledgeable of and adhere to all pertinent federal, state, and local laws, standards of the Ohio Department of Mental Health, and appropriate professional ethical standards. Two years experience preferred.
PHYSICAL REQUIREMENTS: Must be able to lift up to 25 lbs.
