Community Services Care Coordinator (CSCC) - Clinical

Belong Health is seeking a Community Services Care Coordinator-Clinical to provide administrative support to the care team and ensure that members receive supportive services for quality of life. Ideal candidate will have a social/health sciences or business degree, 2+ years experience in care coordination or customer service, and excellent problem-solving skills.

Responsibilities

Orient new members to the Belong Health DSNP program
Perform assessments such as health risk assessments and social determinants of health screening
Provide input in the development of the member’s individualized care plan (ICP)
Provide ongoing support to members related to coordination and access to services
Facilitate and monitor implementation of member care plan
Assist with documentation and care team meetings
Identify and resolve member service and retention related issues
Ensure compliance with clinical quality measures and standards

Education

Bachelor’s degree in social/health sciences or business
