Responsibilities
Conduct daily telephonic outreach to patients and families
Field inbound calls from prospective and existing patients
Schedule and confirm patient appointments
Support patients and families with necessary paperwork
Connect with individuals to determine eligibility for services
Perform various clerical or administrative functions
Attend team meetings and participate in ongoing training
Problem-solve and organize time efficiently
Education
Some College Courses or an Associates degree
