Nurse Practitioner - Home Visits - Per Diem

ComplexCare Solutions is seeking a passionate Nurse Practitioner to conduct in-home and virtual assessments for health plans, focusing on driving better clinical outcomes and improving member engagement.

Skills

Knowledge of CMS Regulations and NCQA HEDIS Guidelines
Familiarity with ICD-9, ICD-10 and CPT-4 coding
Effective communication with elderly and chronically ill patients
Ability to multitask and provide excellent customer service

Responsibilities

Complete comprehensive member population reviews and documentation
Support physician-patient relationship and communicate assessment findings
Recommendations to improve members' knowledge of chronic conditions
Address identified gaps in care and escalate urgent situations
Maintain compliance with policies, procedures, and confidentiality requirements

Education

Active un-encumbered nursing license
Nurse Practitioner Certification from ANCC or AANP
Current CPR certification
