Nurse Practitioner - Home Visits - Per Diem
ComplexCare Solutions is seeking a passionate Nurse Practitioner to conduct in-home and virtual assessments for health plans, focusing on driving better clinical outcomes and improving member engagement.
Skills
- Knowledge of CMS Regulations and NCQA HEDIS Guidelines
- Familiarity with ICD-9, ICD-10 and CPT-4 coding
- Effective communication with elderly and chronically ill patients
- Ability to multitask and provide excellent customer service
Responsibilities
- Complete comprehensive member population reviews and documentation
- Support physician-patient relationship and communicate assessment findings
- Recommendations to improve members' knowledge of chronic conditions
- Address identified gaps in care and escalate urgent situations
- Maintain compliance with policies, procedures, and confidentiality requirements
Education
- Active un-encumbered nursing license
- Nurse Practitioner Certification from ANCC or AANP
- Current CPR certification
