Nurse Practitioner (NP) / Physician Assistant (PA)

Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.The NP/PA will work in a mobile practice, primarily serving a long-term care setting.Essential Functions• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form• Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients• Orders and reviews diagnostic and therapeutic tests relative to patient's age-specific needs• Assists with the provision of care in accordance with facility, state, and federal regulations• Monitors the effectiveness of interventions• Maintains relationships with all appropriate contacts in facilities• Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care• Assists with the development and maintenance of physician relations, as assigned• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive mannerQUALIFICATIONS:• Bachelor's Degree required• Degree from an accredited Nurse Practitioner or Physician Assistant Program required• Current NP License required• BLS - Basic Life Support through American Heart Association required• ACLS - Advanced Cardiac Life Support through American Heart Association preferred• Current Infection Control Certification required• Board Certified/Board Eligible required• Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required• COVID and Flu Vaccine preferred• 1 year of NP experience preferred
