Outreach Case Manager

The Outreach Case Manager reports to the Shelter Manager or Navigation Services Manager to work with those who are unsheltered. This position is responsible for helping assist people move from unsheltered living situations to emergency shelters and stable housing. The outreach teams regularly visit known populated camps to build relationships with those who are unsheltered. The Outreach Case Manager will collaborate with other community partners and participate in joint outreach efforts to assist people in getting housing assessments, sign up for Oregon Health Plan (OHP), acquire documentation needed to get housed, and help those who are unsheltered get connected to local resources. The Outreach Case Manager will focus on one or more of the following areas: Outreach Case Management for adults,  case management and outreach for singles, couples, and families, and/or community health work and health and housing integration.


Wage: $25.13 + DOE + DOQ

Hours: Monday - Friday 8am - 4:30pm


Preferred Qualifications:
  •  Associate Degree in human services, social work, sociology preferred. Can substitute years of experience for degree requirements. 
  • Two years of case management or social services experience; and/or equivalent combination of education and experience preferred.
  • Knowledge of and experience in using trauma-informed care principles required. 
  • Demonstrated experience in mediation between persons exhibiting escalated behavior required.
  • Must have valid Oregon Drivers license; must have been a licensed driver for at least 2 years; must have a driving record that permits coverage under C@P’s company insurance.


Essential Job Duties Include (But Not Limited To):
  •  Assist unsheltered individuals and families by providing housing navigation. 
  • Support unsheltered individuals to take the next step in stability through accessing resources, including Oregon Health Plan, domestic violence intervention, drug and alcohol treatment, behavioral and mental health services, and others.
  • Coordinate health integration at existing shelters, including, but not limited to, the managing of scheduling and connecting people to appointments. 
  • Network and attend meetings with other social service agencies to promote interagency cooperation and coordination. 
  • Participate in the collaborative outreach efforts alongside partnering organizations, including Salem Housing Authority, HIV Alliance, Easter Seals, ARCHES, and Northwest Human Services. 
  • Attend required training, team meetings, and learn policies and procedures for CAP operations. 
  • Enter data in HMIS and assure data quality


$25.13 - $25.13 an hour

Work environment: Close quarters with vulnerable people including individuals without stable living situations who may be struggling with mental health and substance use concerns. Indoor/Outdoor work environment with frequent interruptions and demands, and occasional unpredictable behavior. Frequent walking through camps, which may have unstable surfaces (mud, roots and rocks on ground). This position requires walking long distances outdoors in adverse weather conditions. Must be able to push, pull, and/or lift 15 pounds or less. Driving between Church at the Park operated and managed sites.
