Nurse Practitioner OH (Cleveland) HYBRID apx 3 day virtual/2 days home visits

Skills

Board-Certified Nurse Practitioner or Physician’s Assistant
Experience in providing clinical services to individuals with chronic medical and behavioral health conditions
Proficiency in clinical nursing skills
Strong organizational and problem-solving skills
Ability to work independently and as part of a team

Responsibilities

Provide comprehensive care management for members
Perform urgent medical/behavioral health visits and telephone calls for members
Conduct home visits for panel members
Interface with specialists, hospitals, and community-based organizations
Attend team meetings and case conferences
Assist with the development and enhancement of clinical programs

Education

AANC or AANP certification for Nurse Practitioners
Active DEA Controlled Substance Registration or eligible for application
Certified in Basic Life Support for Healthcare Providers
