Position Overview:
We are seeking a dedicated Health Professionals with experience in patient transportation coordination, treatment placement, and disability Insurance claims. This role is essential to ensuring that patients receive timely, appropriate care while managing Insurance claims and paperwork with efficiency and compassion. The ideal candidate will be skilled in patient advocacy, knowledgeable about healthcare procedures, and experienced in disability claim processes.
Responsibilities:
Patient Transfer Coordination
-Organize and coordinate patient transfers across healthcare facilities as needed.
-Ensure that all required documents, permissions, and patient information are prepared for a smooth transfer process.
-Work with medical staff to secure transportation, accompany patients if needed, and ensure continuity of care during transfers.
Treatment Placement:
-Assess patient needs and identify appropriate facilities or specialists to continue their care.
-Serve as a liaison between patients, healthcare providers, and administrative staff to arrange necessary appointments, admissions, or referrals.
California Disability Claims Assistance
-Guide patients through the disability claim process, including documentation and application requirements.
-Assist patients in gathering and submitting all required paperwork for disability benefits.
-Collaborate with relevant agencies and healthcare professionals to support accurate, timely claim submissions.
Patient Advocacy:
-Provide empathetic support to patients and their families, addressing any concerns or questions.
-Maintain patient confidentiality and adhere to all regulatory guidelines, including HIPAA and other relevant policies.
Qualifications:
-Hold a valid and unrestricted Health professional license in the state of California
-Must Hold a current California State Disability Insurance (SDI) Online Account
-Minimum of 2 years experience in healthcare coordination, patient transfer, or disability Insurance claims.
-Knowledge of state and federal disability claim processes and regulations.
-Exceptional organizational and interpersonal skills, with a strong focus on patient care and advocacy.
-Ability to work independently and as part of a multidisciplinary team.
Pay: The annual salary ranges from $118,000.00 to $198,000.00, depending on qualifications and experience.
