Job SummaryThe Caring Together Addictions Case Manager provides directservices, and appropriate documentation for women enrolled in theCaring Together Program. Duties include assessment needs,facilitation of individual and group sessions, collaboration withcorrectional institutions, recovery planning, including treatmentplans and ensuring that all appropriate documentation and clinicalnotes are completed in the patient charts within the requiredtimeframe. Knowledge and acceptance of medication assistedtreatment (MAT) is required. Position reports to the ProgramDirector.The Caring Together Program is located in the East Falls section ofPhiladelphia at 4700 Wissahickon Avenue, Philadelphia PA19144.Essential Functions• Duties include assessment needs, facilitation of individual andgroup sessions, collaboration with correctional institutions,recovery planning, including treatment plans and ensuring that allappropriate documentation and clinical notes are completed in thepatient charts within the required timeframe.• Knowledge and acceptance of medication assisted treatment (MAT)is required.Required Qualifications• Minimum of a Bachelor's degree in Human Services.• Please review the Equivalency Chart for additionalinformation.• Minimum of one year of experience.Physical Requirements• Typically sitting at a desk/table.Location• Philadelphia, PAAdditional InformationThis position is classified as exempt grade J. Compensation forthis grade ranges from $ 47,490.00 to $ 71,230.00 per year.
