Behavioral Health Outreach Care Specialist

Location: This is a virtual position, and the ideal candidate must reside within 50 miles of an Elevance Health PulsePoint location.

Work schedule: Monday - Friday, 9 am - 6 pm MST.

The Behavioral Health Outreach Care Specialist is responsible for conducting behavioral health provider searches for members with complex diagnoses, supporting CMs working with our members, and conducting screening calls to members. 

How you will make an Impact: 

  • Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.

  • Assess member compliance with medical treatment plans via telephone or through on-site visits.

  • Identifies barriers to plan compliance and coordinates resolutions.

  • Identifies opportunities that impact quality goals and recommends process improvements.

  • Coordinates identification of and referral to local, state, or federally funded programs.

  • Coach members on ways to reduce health risks.

  • Prepares reports to document case and compliance updates.

  • Establishes and maintains relationships with agencies identified in the appropriate contract.

  • Conduct outbound telephonic provider searches for behavioral health providers to determine network status and availability utilizing Anthem’s provider finder and other resources.

  • Supports Behavioral Health case management service by responding to inbound calls from members interested in service.

  • Responds to telephone and email inquiries from internal teams and providers

Minimum Requirements:

  • Requires a H.S. diploma or equivalent and a minimum of 1 year related experience; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities, and Experiences:

  • Preferred certification: certified nurse assistant or certified medical assistant and/or BS/BA degree in a related field is preferred.

  • Bilingual skills are a plus.

  • Experience or knowledge of behavioral health and medical is preferred

  • Previous insurance experience is preferred

  • Experience navigating and utilizing multiple computer applications simultaneously is preferred.

  • Experience working in a telephonic environment, call center experience is preferred.

  • Excellent communication skills.

  • Strong computer and keyboarding skills.

  • Experience preparing member cases for the documentation system via data entry, accessing multiple systems, and researching information is a plus.

  • Experience in conducting outbound calls to members to ascertain interest in service is a plus.

  • Prepares member cases for documentation system via data entry, accessing multiple systems, and researching information.

  • Previous insurance experience is helpful.

For candidates working in person or remotely in the below location(s), the hourly* range for this specific position is $20.23 to $31.80.

Locations: Colorado and Nevada.
