Care Coordinator
Evergreen Nephrology partners with nephrologists to transform kidney care through a value-based, person-centered approach. They are looking for a dedicated Care Coordinator to help patients meet their kidney care goals.
Skills
- Interpersonal communication skills
- Exceptional listening abilities
- Compassionate and open-minded
Responsibilities
- Establish trusting relationships with patients and families
- Monitor and coordinate patients' treatment plans
- Educate patients about their condition
- Coordinate care with healthcare providers
- Track patient admissions and discharge follow-ups
- Link patients to community resources
Education
- Some clinical experience (EMT, MA, CNA, or Phlebotomy certification preferred)
