Nurse Care Manager (Remote IDT)
Evergreen Nephrology seeks a compassionate Nurse Care Manager to enhance kidney care through patient-centered strategies. Join a dynamic team focused on improving outcomes for patients with kidney disease.

Responsibilities
- Build empathetic relationships with patients
- Evaluate patients’ needs and translate them into clinical needs
- Interface with various health care professionals to develop case management plans
- Monitor care and recommend alternative levels of care
- Educate patients and families on treatment options
Education
- Current RN License is required
- BSN preferred
