Obesity Medicine Physician
FORM Health is seeking an obesity medicine physician to provide longitudinal telehealth care for patients with obesity and collaborate with a multidisciplinary team towards exceptional care.
Skills
- MD degree and board certified
- Diplomate of the American Board of Obesity Medicine
- Licensed to practice and prescribe legally in Pennsylvania
- Licensed & credentialed with insurance payers in one or more of the following additional states: IL, MA, TX, or NY
- Excellent interpersonal skills
- Prior experience in telemedicine visits
Responsibilities
- Provide longitudinal telehealth care for patients with obesity
- Collaborate with advanced practice professionals and registered dietitians
- Deliver evidence-based obesity treatment through telemedicine
Education
- MD degree
- Board certification
- Diplomate of the American Board of Obesity Medicine
